Home and Community-Based Alternatives (HCBA)-Seniors with Disabilities
Description
The Home and Community-Based Alternatives Waiver (HCBA) program helps put supportive services in place that allow participants to stay at home.
A social worker and nurse work together to determine the services clients need. The client, family and caregiver partner with the care team to plan and carry out the care management plan. The team has regular contact to help arrange, coordinate and monitor recommended medical and community services, including nursing care and some personal care services.

Linkages Care Management -Case Management Services for Seniors
Description
The Linkages Social Worker works with the you to create a plan based on the goals you identify to improve your life situation. The Linkages Social Worker helps to create action steps and track those goals during a 6 month to 1-year period. The Linkages Social Worker will stay in contact by telephone and will make in-home visits.
The Multipurpose Senior Services Program (MSSP) Assists Sonoma County residents 65 or older who would rather live at home than in a nursing facility. The care manager aNd nurses work with each client to find out what they want and need, then create a plan to help them meet their goals. When new challenges arise, the MSSP team will help figure out options and navigate concerns. Services offered can be in-home support, meal delivery, transportation, and caregiver respite for seniors 65 and older who wish to remain in their homes.

Petaluma Peer Recovery Center seeks to build a community that fosters growth and recovery through connection with peers. They respect individual pathways to wellness and value self-direction and mutual support. Offers various services to support seniors facing mental health or substance use challenges. While not exclusively for older adults, it provides inclusive programs that can benefit seniors.
The Senior Center offers programs and classes that reflect the social, physical, intellectual, and psychological interests of our aging population – which promote dignity, self-respect, a sense of purpose, inclusion and diversity to our community.
The Rohnert Park Senior Center hosts daily lunches, social gatherings, and maintains a gift shop featuring local artisans. The wellness programs include fitness classes and health screenings.
